Facility Analyze > Destinations by TEAM Episode Category
What: Patient destinations for each TEAM Episode Category have been added to the existing Hospice Destinations table with additional options included in the existing drop down filter
Why: Users can now see which post-acute providers are destinations for patients in each TEAM Episode Category. This enables more strategic targeting and partnership development aligned to specific episode types.
MA Contracts: Hospital Patient Utilization Trends
What: A new Hospital Patient Utilization Trends table has been added to the Utilization tab within MA Contract Details, showing inpatient utilization for MA contract enrollees from 2019-2023
Why: Users can now analyze historical inpatient utilization trends for a selected MA contract, supporting contract evaluation, strategic planning, and partnership discussions.
Hospice Cost of Care Impact Added to SNF Facility Analyze pages
What: The Hospice Cost of Care Impact chart and Utilization table are now available on the SNF Analyze page.
Why: This enhancement allows users to demonstrate how earlier hospice utilization can reduce total Medicare spend and improve patient care outcomes. The data supports more informed conversations with physicians and hospitals to strengthen referral relationships.

Why: It helps organizations identify, prioritize, and grow high-value FFS referral relationships across all agencies by revealing where volume is coming from and where opportunities exist.
CMS TEAM Episode Category grid - new category called "Average Total Spend Per Patient (Inpatient + 30 Days Post Discharge)
What: Metrics to show the average total spend per patient for each TEAM episode category, for the applicable hospital, state, and county
Why: Compare spend per patient, by TEAM category, at the selected hospital to the state and county averages

What: This new hospice feature provides a visual analysis showing how earlier and longer hospice enrollment correlates with lower overall Medicare spend and reduced healthcare utilization in a patient’s final months of life.
Why: To help our clients prove the value of early hospice to their referral sources - this new tool enables data-driven decisions to optimize hospice care and resource use.
Adding additional Metrics to Inpatient MS-DRG Breakout table
What: Helps hospices uncover more opportunities for engagement and hospital referrals with with additional Metrics added to this grid
Why: This updated inpatient MS-DRG utilization and outcomes view shows excess hospital days, post-discharge hospice starts, and mortality by DRG, enabling hospice teams to pinpoint high-acuity conditions with the greatest opportunity for earlier, more appropriate hospice engagement.
