What: Two new metrics now show which HOS receives the most patients from each facility referral source, along with the percentage of those referrals.
Why: Instantly identify the strongest HOS affiliation for each referral source—no need to navigate to the Analyze page.

Hospital Explore > Add TEAM Participant Metric and Filter
What: Allows users to filter the list to hospitals participating in the CMS TEAM Model program
Why: To learn which hospitals PAC's should focus on for this program
Hospital Explore > Add TEAM Episode Category Metric and Filter
What: Allows users to understand the patient volumes for a TEAM Episode Category regardless of participation. The metric will be blank until the user selects the Episode category.
Why: To identify the highest opportunity to pair with a hospital for referrals
Facility Analyze > Add TEAM Episode Category Table
What: Allows users to understand the number of patients, discharges, hospital ALOS, and readmission rates for each TEAM Episode category at each hospital, regardless of participation
Why: To improve relationships with the hospital referral sources and to ensure success with the TEAM program
